Below you will find links to forms that we use in the process of setting up your account and accomplishing your goals of doing more business with Fleet Street money.

These forms can be downloaded, filled out and emailed, faxed or mailed for immediate action.

Fleet Street Application Form

This form is used to apply for an account with Fleet Street.  Setting up an account now is considered good planning, without penalty, because with Fleet Street:

  • There are no minimum monthly charges
  • No yearly contracts with minimum fees
  • No hidden surprises or charges
  • You only pay for the exact number of days the money is out

This enables you to be prepared for the possibility that our services are required, and to then act upon those needs quickly.

This form can be filled out online, then saved to your hard drive as a Word document.  You can then print the form, sign it and fax it to 416-929-1570 or mail it, or you can save the completed form and attach it to an to our office.  We can begin processing the application and obtain signatures and the additional documents later after it is approved.

Letter of Direction

This form is used to provide authorization and instruction to your creditors regarding the change of accounts receivable management to Fleet Street Financial.

This form can be filled out online, then saved to your hard drive as a Word document.  You need to copy this information onto your letterhead, then print the form, sign it and fax it to 416-929-1570 or mail it, or sign it when visiting our office.

Finders Agreement

This form is used to compensate for referrals of clients to Fleet Street Financial.  For more information, go to the Brokers page.
